A 4-year-old female Golden Retriever, with a history of whelping 9 days ago, was presented for evaluation of a hemorrhagic discharge from the vulva.

Sonographic Differential Diagnosis

Severe metritis. Placentitis. Regional peritonitis. Potential mummified fetus with secondary placentitis or metritis is suspected.

Image Interpretation

The ovaries were normal. The left ovary measured 0.9 cm. The right ovary was mildly enlarged and measured 1.0 cm. The uterine body measured 1.01 cm with a slight amount of fluid accumulation noted. Minor wall thickening was noted. The left horn of the uterus was enlarged and thickened with an irregular wall and focal tissue thickening. Regional inflammation was noted. Transmural inflammation was noted.

Outcome

None

Comments

Ovariohysterectomy is strongly recommended. If medical therapy is chosen, then aggressive broad spectrum antibiotics and IV fluids would be recommended, with sonogram every 48 hours (assuming that the patient is making positive progress to ensure adequate resolution). PGF2-alpha (lutalyse) treatment should also be considered if medical therapy is chosen.
