 A 5-year-old NM DSH was presented for evaluation of vomiting and weight loss. Abnormalities on serum biochemistry were low BUN and elevated ALP (309) activity and bilirubin.

Sonographic Differential Diagnosis

Enlarged liver with free fluid. Suspect hepatic lymphoma or lipidosis. A lesser possibility is suppurative hepatitis.

Image Interpretation

The liver was diffusely hyperechoic to the falciform fat. Mildly increased portal markings were noted. A minor amount of free fluid was noted between the liver lobes. The common bile duct was unremarkable. The gallbladder wall was double layered.

Liver – lipidosis, cholangio-hepatitis complex, FIP, lymphoma
Cholangitis
Pancreas – pancreatitis

Sampling

FNA of the liver and free fluid was obtained. The liver cells demonstrate classical fatty liver. This is likely metabolic and often accompanies pancreatitis or possible enteritis according to the literature and my experience. The fluid supports vascular leakage from some location. This could be through the liver capsule or possibly from pancreatitis or other organs in the cavity. There is no suggestion of neoplasia or sepsis.
