Multicentric neoplasia in a 5 year old FS ferret

A 5 year-old female spayed ferret was presented for an episode of lethargy which had improved overnight. On exam, there was a palpable abdominal mass. Blood work was unremarkable. 

A 5 year-old female spayed ferret was presented for an episode of lethargy which had improved overnight. On exam, there was a palpable abdominal mass. Blood work was unremarkable. 

Sonographic Differential Diagnosis

Multicentric neoplastic pattern involving spleen, lymph nodes and liver with hepatic lymphadenopathy. Multicentric lymphoma is suspected.

Image Interpretation

The spleen contained multiple target lesions. It exhibited generalized enlargement and scalloping contour, which was strongly suggestive for infiltrative disease. Free fluid was noted around the spleen as well. The liver was largely uniform, slightly coarse, and contained minor increased portal markings. A hypoechoic nodule was noted in the caudate process of the liver as well. Gallbladder unremarkable. Surrounding free fluid noted. The portal vein was deviated due to hepatic lymphadenopathy (approximately 1 cm each) as well as caudate process expansion. The mesenteric lymph nodes were also enlarged. There was hyperechoic surrounding fat and reactive mesentery noted throughout the mid-abdomen.

Clinical Differential Diagnosis

Neoplasia: lymphoma, carcinoma, adrenal, pancreatic; Lymphadenopathy: granulomatous, neoplastic; Cystic disease: renal, pancreatic, hepatic.

Sampling

None. Fine needle aspirates of spleen and mesenteric node and liver would be ideal for further definition with initiation of chemotherapeutic protocol.
