Multiple myeloma diagnosed by jejunal lymph node aspirate/cytology in a 8 year old DSH cat

An 8-year-old feline DSH was presented for evaluation of weight loss. Abnormalities on serum biochemistry were severe hyperglobulinemia (globulin 91,) elevated bilirubin and fPL. Chronic bronchitis was evident on survey radiographs.

Sonographic Differential Diagnosis

Multi centric, regional lymphadenopathy mesenteric root and infiltrative splenic pattern. There is a strong suspicion for multiple myeloma or lymphoma or similar neoplasia. Possible chronic infectious with reactive lymphadenitis. Hyperechoic surrounding fat. FNAs were performed of the spleen and lymph node.

Image Interpretation

The proximal colon was slightly thickened and measured 0.3 cm. Jejunal lymph node mass was noted in this patient and measured 3.5 cm. Caudal mesenteric lymph node was also mildly enlarged and slightly thickened with heterogenous parenchymal changes. The spleen was enlarged and micronodular measuring 1.13 cm. This is strongly suspicious for infiltrative disease. Rapid view of the heart revealed no evident pathology.
