Case Study

Sonographic Differential Diagnosis

Linear foreign body obstruction from pylorus to the jejunum. A tethered portion of jejunum was noted due to linear foreign body. Significant regional inflammation was noted. Underlying, chronic inflammatory disease may also be present owing to some loss of detail and thickening in the variable portions of the affected small intestine. Guarded prognosis depending upon the outcome from surgery and underlying gastrointestinal histopathology. Resection and anastamosis will likely be necessary in this case.

Image Interpretation

The stomach was filled with foreign matter that continued into the distal small intestine. This created an obstructive pattern. The material appeared to be anchored into the pylorus and continued into the jejunum. The distal small intestine was dilated. The obstructive pattern was followed by empty small intestine. Reactive fat was noted associated with the pylorus and variable portions of the small intestine.
