A 12-year-old female spayed West Highland White terrier dog was presented for evaluation of pollakiuria and stranguria of 4 weeks duration. Urinalysis showed an inappropriately low SG (1.015), blood, and large number of epithelial cells. Blood work showed mildly elevated ALP activity and CK.

Sonographic Differential Diagnosis

Non-resectable dorsal trigonal bladder mass with potential urethral involvement. Regional inflammation. Left hydroureter, minor left hydronephrosis.

Image Interpretation

The urinary bladder presented a dorsal complex mass that measured 2.9 x 2.55 cm with mixed echogenic changes and variable wall thickening. The left ureter was obstructed by the polypoid mass. The bladder mass was significantly mineralized. Regional, fatty inflammation was noted and was associated with the cystourethral junction. Minor urethral thickening was noted. The iliac bifurcation appeared free of evident pathology. Slight pyelectasia was noted in the left kidney with hydroureter at 0.55 cm. Slight mineralization was noted in the left kidney. The capsules were acceptably uniform for this age patient without dramatic irregularities. The right kidney measured 4.05 cm. The left kidney measured 4.68 cm.
