A 5-year-old female spayed ferret was presented for evaluation of lethargy, weakness, and large bowel diarrhea. Blood work showed hypoglycemia and elevated ALT and GGT activity.

A 5-year-old female spayed ferret was presented for evaluation of lethargy, weakness, and large bowel diarrhea. Blood work showed hypoglycemia and elevated ALT and GGT activity.

Sonographic Differential Diagnosis

Lymphoproliferative presentation involving epigastric lymph node, spleen and splenic lymph node. Hepatic lymphadenopathy. Lymphoma is a primary differential. Occult insulinoma can also be present given the patient’s history.

Image Interpretation

The spleen was enlarged with an infiltrative pattern, irregular contour and splenic lymph node enlargement measuring 0.54 cm. The spleen at maximum width measured approximately 2.0 cm. Micronodular changes were noted. The hepatic lymph nodes were enlarged at 1.23 x 0.47 cm. Epigastric lymph node was enlarged and measured 0.93 x 0.48 cm. Cranial mediastinal lymph node was enlarged and measured 1.67 x 0.77 cm. Reactive mesentery was noted throughout the abdomen.

Clinical Differential Diagnosis

Liver disease (neoplasia, acute hepatitis). Hypoglycemia (insulinoma, sepsis, liver disease).

Sampling

FNAs were performed without complication. Cytology revealed splenic hyperplasia and extramedullary hematopoiesis with a reactive lymph node.
