A 14-year-old MN DSH cat was presented for evaluation of weight loss. Survey radiographs and urinalysis were both within normal limits. Laboratory testing showed mild hyperglycemia, hypoalbuminemia, and mild anemia.

DX

Annular colonic mass, carcinoma vs lymphoma

Sonographic Differential Diagnosis

Annular colonic mass with slight regional lymphadenopathy. The primary differentials include colonic carcinoma versus colonic lymphoma with a minor potential for granulomatous disease.

Image Interpretation

The jejunum revealed minor, increased submucosal echogenicity with hypertrophied muscularis. The changes were considered minor. The colon in this patient presented an annular mass that measured 3.5 cm in diameter with focal areas of mineralization. The colonic mass is in the proximal descending colon. The mass is partially obstructive with stool impaction. The mass appears resectable. However, regional lymphatic spread is suspected. Regional colonic lymph node was enlarged and heterogenous with mixed, hypoechoic changes. This is strongly suggestive for metastatic disease given that the echogenicity and structural deviations are similar to that of the colonic mass. However, FNAs are recommended for confirmation. The lymph node measured 2.2 x 1.22 cm.

Comments

The colonic mass appears resectable with a subtotal colectomy, and appropriate chemotherapy is recommended.
