A 10-year-old neutered male Maine Coon cat with a history of mega-colon was presented for evaluation of passing smaller stools, vomiting bile, and decreased appetite. On physical examination, a palpable abdominal mass was present.

Sonographic Differential Diagnosis

Bilateral renomegaly with target lesions. This is strongly suggestive of renal lymphoma or similar.

Image Interpretation

The kidneys in this patient were significantly enlarged with multiple nodular changes, and large and small target type lesions throughout the cortices. Parenchymal renomegaly was noted in the right kidney and measured 6.52 cm with loss of corticomedullary detail and expansion of the cortex with subcapsular hypoechoic capping. The left kidney presented significant subcapsular capping and measured 6.05 cm. Pericapsular, hyperechoic inflammatory pattern was also noted.

Bilateral renomegaly, renal lymphoma most likely

Clinical Differential Diagnosis

Abdominal mass – stool in colon (megacolon), mass effect of any organ system (neoplasia, abscess, granuloma, cyst), hydronephrosis.

Sampling

Ultrasound-guided FNA of the kidneys is strongly recommended.
