A 7-year-old spayed female English bulldog was presented for lethargy, shaking and anorexia. Physical examination was within normal limits. Radiographs revealed pulmonary edema and gas patterns in the stomach and gastrointestinal tract.

Sonographic Differential Diagnosis

Blood pressure and coagulation panel with Plavix therapy, gastrointestinal protectants and broad spectrum antibiotics should be considered in this patient. However, given the aggressiveness of the right adrenal gland mass, which is suspicious of pheochromocytoma, the prognosis long term is very poor.

Image Interpretation

The right adrenal gland mass has significantly progressed from 3.8 x 2.5 cm to 4.37 x 2.5 cm with vascular invasion into the vena cava. The left adrenal gland was mildly prominent and irregular as well. There is a thrombus at the splenic vessel in the region of the gastric fundus which continued into the midabdomen. No blood flow was noted in this region.

DX

Right adrenal gland mass with invasion of caudal vena cava. Splenic thrombus.

Outcome

The patient was started on antibiotics and IV fluids.

Clinical Differential Diagnosis

Pulmonary edema – cardiac disease, ARDS, post seizure, post upper airway obstruction, possibly pneumonia.

Sampling

Cytology of abdominal fluid revealed that the effusion was an exudate. No infectious agents were identified and no evidence of neoplasia was seen. Culture and sensitivity revealed suspicion of bacterial growth.
