A 16-year-old FS DLH cat presented for evaluation of grade 3/6 murmur that had developed since examination 6 months ago. There were no overt clinical signs of heart disease.

A 16-year-old FS DLH cat presented for evaluation of grade 3/6 murmur that had developed since examination 6 months ago. There were no overt clinical signs of heart disease.

Sonographic Differential Diagnosis

Largely normal heart. The murmur may be due to dynamic obstruction of the right ventricular outflow tract (DRVOTO). It`s possible that the murmur was not previously ausculted due to its dynamic origin (it may be intermittent).

Image Interpretation

There is very mild mitral insufficiency present, but this cannot cause any audible heart murmur. Color Doppler flow across the RVOT and LVOT are unremarkable, but the continuous wave flow profile across the RVOT shows a somewhat dagger-shaped pattern, which can be an indicator of dynamic obstruction of the right ventricular outflow tract (DRVOTO).

DX

Dynamic obstruction of the right ventricular outflow tract (DRVOTO), mild MI

Outcome

Recheck the cat after one year.

Clinical Differential Diagnosis

Mitral/tricuspid endocardiosis, vegetative endocarditis, SAM, dilated/hypertrophic cardiomyopathy, hypertrophy secondary to systemic hypertension.
