A 6-year-old NM Maltipoo was presented for evaluation of a recent onset systolic murmur without any overt clinical signs. Systolic blood pressure was elevated (155), however, the patient was stressed.

A 6-year-old NM Maltipoo was presented for evaluation of a recent onset systolic murmur without any overt clinical signs. Systolic blood pressure was elevated (155), however, the patient was stressed.

Sonographic Differential Diagnosis

Mild DVD (mitral and tricuspid valve), mild mitral and tricuspid insufficiency, no volume overload, no atrial enlargement I would not start with any therapy in this patient (you can administer nutriceuticals if the owners want to give something). The patient should be rescanned after 6 month for detection of enhanced left ventricular volume load or/and left atrial enlargement.

Image Interpretation

The patient has normal left and right ventricle, a normal left and right atrium (not enlarged), a mildly thickened mitral valve which is slightly prolapsing into the left atrium and a slightly degenerated tricuspid valve. CDI reveals mild mitral and tricuspid insufficiency. Flows across the LVOT and RVOT are normal in terms of maximum velocity and without any turbulence. There is no sign of pulmonary hypertension present in this case.

DX

Mildly thick MV, slight MVP, slight TV degeneration. Mild MI, mild TI.

Clinical Differential Diagnosis

Mitral/tricuspid endocardiosis, vegetative endocarditis, ventricular hypertrophy secondary to systemic hypertension, pulmonary hypertension
