A 16-year-old FS DSH with a history of hyperthyroidism, hematuria, heart murmur, and chronic renal disease was presented for evaluation of inappetence. Hypertension was present (160). BUN was elevated (75) but decreased to 46 after IV fluids.

Sonographic Differential Diagnosis

Thyrotoxic cardiomyopathy with myocardial fibrosis. The thyroid was unregulated. No specific cardiac medications are recommended at this time.

Image Interpretation

The cardiac presentation in this patient presented stiff, hypercontractility with normal internal volumes and minor increased wall thicknesses of the septum and free wall. The internal diameter of the left ventricle was normal with mild to moderate myocardial remodeling. The left atrium was normal. Turbulence of the left ventricular outflow tract and mitral valve were noted. This is likely the source of the murmur.
