A 16-year-old Abyssinian was presented for PU/PD and anorexia. On physical examination weight loss was present. The only abnormalities on urinalysis and blood chemistry were isosthenuria and azotemia, respectively. The patient was treated with I.V. fluids, which resulted in an improvement in the azotemia after three days.

Sonographic Differential Diagnosis

Diffuse, multi-focal target lesions in the liver. Suspect neoplasia. Potential for abscessation or nodular hyperplasia. Guarded to poor prognosis.

Image Interpretation

The liver presented multiple target type expansive nodules that were sampled by fine-needle aspirate. This is consistent with neoplasia such as lymphoma, carcinoma, or possible abscessation or nodular hyperplasia.

DX

Liver: metastatic tumor with foci of active inflammation.

Outcome

The patient was humanely euthanized 5 days post diagnosis owing to a declining clinical condition.

Clinical Differential Diagnosis

Renal disease – chronic kidney disease, pyelonephritis, neoplasia, hydronephrosis, renoliths

Sampling

US-guided FNA cytology of the liver was suggestive of a metastatic tumor with foci of active inflammation.
