Bladder mass in an 11 year old FS Vizsla

Case Study

Bladder mass in an 11 year old FS Vizsla

An 11-year-old FS Vizsla was presented for diarrhea, tenesmus, and stranguria for a few months. Rectal exam revealed minor thickening in the urethral region. A Canine diarrhea panel was positive for Clostridium Perfringens Enterotoxin A.

Sonographic Differential Diagnosis

Cystourethral bladder mass, apical polyp.

Image Interpretation

The urinary bladder presented a 4 cm pelvic urethral mass with thickening at the cystourethral junction. No obstruction of the ureters was noted at this time. A separate apical polyp was also present, which is thought to be associated with the transitional cell carcinoma and is not overtly resectable. The mass, however, would likely be amenable to ultrasound guided laser ablation or UGELAB. The ureters were not visualized and assumed to be normal.

DX

Bladder mass.

Outcome

Ultrasound guided laser ablation of the urethral mass was recommended. Three-view thoracic radiographs and potential echocardiogram were also advised to ensure no thoracic pathology was present.

Clinical Differential Diagnosis

Stranguria – bladder pathology (neoplasia, uroliths, granulomatous cystitis, chronic cystitis), urethral pathology (neoplasia, uroliths, granulomatous inflammation). Tenesmus – colon pathology (neoplasia, colitis, stenosis), pelvic disease (neoplasia, inflammation, lymphadenomegaly).
