Pheochromocytoma of the left adrenal gland in a 12 year old SF boxer mix

Case Study

Pheochromocytoma of the left adrenal gland in a 12 year old SF boxer mix

A 12 year-old SF boxer mixed breed dog was presented for lethargy and drooling for approximately 5 days. Abnormalities on blood chemistry were elevated ALP activity, amylase, lipase, and CPL.

Sonographic Differential Diagnosis

Left adrenal gland mass. Appears resectable at this time and is likely pheochromocytoma.

Image Interpretation

The right adrenal gland was normal in size and contour. It measured 0.8 x 3.1 cm and was uniform without evidence of pathology. The left adrenal gland revealed a bright mass that was heterogenic and had mixed hyperechoic and hypoechoic changes. However, it remained within the capsule and measured 5.3 x 3.0 cm. There was no evidence of invasion into the vena cava or phrenic vein. Therefore, surgical resection is suggested. Primary differentials for left adrenal gland mass include pheochromocytoma, non-functional adenocarcinoma or minor possibility of hyperplasia. There was no other structural evidence of disease that would suggest other pathology that would be responsible for the clinical signs.

DX

Pheochromocytoma of the left adrenal gland

Outcome

Left adrenalectomy.

Clinical Differential Diagnosis

Pancreatic disease – pancreatitis, neoplasia. Esophagus – esophagitis, neoplasia, foreign body. Liver disease – inflammatory, infectious, toxic, metabolic, neoplasia. Post-hepatic disease – bile duct obstruction, cholecystitis, mucocele.

Sampling

Left adrenalectomy. Histopathology revealed a pheochromocytoma.
