Gastric mass in a 14 year old FS DSH cat

A 14-year-old FS DSH was presented for vomiting over the last two weeks and hematuria. On urinalysis 3+ protein, 3+ blood and leukocytosis was evident. Abnormalities on CBC and serum biochemistry were neutrophilia and elevated ALP and elevated ALT activity. T4 was elevated (4.6).

A 14-year-old FS DSH was presented for vomiting over the last two weeks and hematuria. On urinalysis 3+ protein, 3+ blood and leukocytosis was evident. Abnormalities on CBC and serum biochemistry were neutrophilia and elevated ALP and elevated ALT activity. T4 was elevated (4.6).

Sonographic Differential Diagnosis

Gastric mass with epigastric lymphadenopathy, non-resectable. Probable infiltrative neoplasia such as LSA, MCT or carcinoma. Possible granulomatous disease. Separate bladder mass or polyp -resectable.

Image Interpretation

The stomach presented a 2+cm cranial wall mass with epigastric lymphadenopathy (9 o`clock position). The lesion is non resectable owing to the pyloric involvement unless a Billroth procedure is considered. The urinary bladder presented a 2 cm dorsal trigonal mass or potentially a polyp which was not obstructive at this time. The remainder of the bladder was uneventful.

DX

Gastric mass with epigastric lymphadenopathy. Bladder mass.

Outcome

Patient was lost to follow-up.

Comments

The epigastric lymphadenopathy suggests localized metastasis there fore a medical approach is likely warranted

Clinical Differential Diagnosis

Vomiting – GI tract pathology (neoplasia, IBD, partial obstruction, infectious- bacteria, fungal, helminths, protozoa), dietary; Liver pathology (cholangitis-cholangiohepatitis, neoplasia, infectious, lipidosis); Pancreatic pathology (pancreatitis, neoplasia); elevated T4- hyperthyroidism; Hematuria – Bladder pathology (neoplasia, cystitis -sterile, non-sterile, uroliths).
