A 10-year-old MN Bichon Frise was presented with a history of prostatomegaly during rectal palpation and a painful lumbosacral spine. CBC and blood chemistry analyses were normal. No urinalysis was performed

Sonographic Differential Diagnosis

Suspected bony infiltration or paraneoplastic pain related to prostatic carcinoma.

Image Interpretation

The prostate was hypoechoic with focal calcifications and a small cystic region. This is consistent with prostatic carcinoma.

Outcome

The patient was referred for an oncology consultation.

Clinical Differential Diagnosis

Prostatomegaly – benign prostatic hyperplasia, cystic prostatic disease, neoplasia, abscess. Spinal pain – secondary to prostatic disease, disc disease, trauma, neoplasia, LS instability.

Sampling

US-guided FNAs of the prostate revealed carcinoma with marked necrosis and chronic inflammation.
