Gastric adenocarcinoma in a 12 year old MN Keeshond

A 12-year-old neutered male Keeshond dog was presented for chronic, intermittent and progressive vomiting of yellow to brown liquid and partial anorexia over a 1-month period. The physical exam revealed low body condition score and moderate halitosis with minor dental tartar. The CBC and blood chemistry analyses were normal. PT and aPTT were within normal limits. The patient was treated unsuccessfully with gastric protectants and antibiotics for 8 days until the sonogram was performed.

Sonographic Differential Diagnosis

This is highly suspicious for neoplasia such as lymphoma, adenocarcinoma or possibility of very aggressive inflammatory event.

Image Interpretation

The stomach had a diffuse 1.8cm thickened gastric wall with variable areas of detail loss. Multiple upper gastrointestinal lymph nodes were enlarged and dramatically hypoechoic.

DX

Gastric adenocarcinoma, metastasis to lymph node.

Outcome

The patient was humanely euthanized 6 days post sonogram due to poor prognosis and clinical decline.

Clinical Differential Diagnosis

Gastritis, ulceration, IBD, gastric foreign body, pyloric outflow delay, neoplasia, pancreatitis, Helicobacter.

Sampling

18-gauge US-guided biopsy of the epigastric lymph node revealed metastatic gastric adenocarcinoma.
