Gastric foreign body in a 6 month old MN Chow Chow with 3 days of persistent vomiting

This 6-month-old MN Chow Chow was presented for persistent vomiting over a 36 hour period. The patient had eaten a large quantity of rawhide chews prior to this episode. The physical exam was normal with minor evidence of dehydration. CBC, blood chemistry, and urinalyses were normal.

DX

Gastric foreign body

Sonographic Differential Diagnosis

Suggestive for pyloric foreign body or other foreign body such as rawhide or cloth. Concurrent intestinal shock bowel.

Image Interpretation

The stomach presented a mild amount of fluid retention with hyper-peristaltic gastric wall which was mildly edematous. A 2.5 cm soft shadowing pyloric structure was noted acting as a cork type presentation within the pyloric outflow. The duodenum and remainder of the intestinal tract appeared normal with a slight amount of intestinal fluid, however no peristalsis was noted. This is most consistent with shock bowel (dilated bowel secondary to hypovolemic shock). Primary foreign body in the pylorus is suspected such as a rawhide or soft cloth type structure.

Outcome

The patient was hydrated and monitored over a 12 hour period. The presumed rawhide chew that was lodged in the pylorus dissolved over a 12 hour period and the patient resumed normal activity without intervention.

Clinical Differential Diagnosis

GI foreign body/obstruction, gastroenteritis, pancreatitis, toxicity.
