Case Study

Colonic Adenocarcinoma in a 12 year old MN DSH cat

This 12-year-old MN DSH was presented for anorexia and weight loss. The physical exam revealed a focally thickened intestine in the mid abdomen but was otherwise normal. The CBC was normal. Blood chemistry evaluation revealed a slightly elevated lipase and slightly elevated triglyceride with moderately subnormal T4. The urinalysis was normal.

Sonographic Differential Diagnosis

Obstructive colonic mass-carcinoma, lymphoma, mast cell disease, complicated colitis with stricture. Appears resectable with subtotal colectomy.

Image Interpretation

Obstructive colonic annular lesion with loss of mural detail. Luminal dirty shadow accumulation suggests an obstructive pattern.

DX

Colonic adenocarcinoma

Outcome

Given the focal nature of the pathology exploratory surgery was selected given the possibility of clean resection without sampling by US-guided methods. The patient recovered uneventfully and was clinically normal for 5 months until a recurrence of the mass was noted on abdominal palpation. This was confirmed sonographically and metastatic disease was noted. The patient was euthanized 2 weeks later after not responding to steroid therapy.

Clinical Differential Diagnosis

GI pathology – Intestinal neoplasia, IBD, pancreatitis, intestinal obstruction, granulomatous disease.

Sampling

Ileocecocolic anastomosis was performed with histopathological analysis revealing colonic adenocarcinoma.
