Intestinal Carcinoma in a 15 year old MN DSH cat

This 15-year-old MN DSH was presented for 3 pounds of weight loss over a 3-month period with normal appetite. Physical exam was unremarkable except for poor body condition. CBC revealed moderate leukocytosis with a left shift and normal chemistry analysis.

Sonographic Differential Diagnosis

Infiltrative neoplastic disease, most likely lymphoma, mast cell, or adenocarcinoma. Other differentials for intestinal neoplasia in the cat include: fibrosarcoma, undifferentiated sarcoma, leiomyoma, leiomyosarcoma, plasmacytoma, and neuroendocrine carcinoma. Given the ultrasound appearance of irregularity, asymmetry and local extension, inflammatory or granulomatous disease were considered less likely.

Image Interpretation

An irregularly shaped, circumferential, hypoechoic mass with focal mineralization is asymmetrically arising from a segment of small bowel wall. The serosal margins of the mass are poorly defined and lobulated extensions into the adjacent omentum and mesentery are suspected. The peritumoral omentum and mesentery are mildly echogenic and may be adhesed. Note the transition from normal to diseased bowel wall on image one. (best seen in video clip) Also on image 1: the linear echogenicity visualized at one o`clock position is a 22 gauge needle.

DX

Well differentiated intestinal carcinoma

Outcome

The patient was lost to follow-up.

Clinical Differential Diagnosis

GI pathology: GI neoplasia, IBD, maldigestion, EPI, extra GI neoplasia, FIP, chronic disease.

Sampling

22g US-guided FNA revealed well differentiated intestinal carcinoma.
