Duodenal foreign body in a 3 year old MN Pug

This 3-year-old neutered male Pug dog was presented for a rapid onset of vomiting, anorexia, and diarrhea. Severe abdominal pain was noted on physical exam. The CBC revealed moderate leukocytosis with a left shift, mild SAP elevation, and mild hypokalemia.

DX

Duodenal foreign body

Sonographic Differential Diagnosis

Duodenal foreign body with resultant gastric stasis secondary to mechanical ileus.

Image Interpretation

A moderately fluid-distended stomach and focally distended proximal duodenal lumen can be seen in video 1. In video 2, an echogenic, translucent structure is present in the duodenal lumen consistent with foreign body made of polyurethane or similar material. Lack of an accompanying distal acoustic shadowing could support soft, porous intraluminal material. In the far field, mild localized omental inflammation is present suggesting an emerging localized peritonitis.

Outcome

The patient responded well to surgery and hospitalization. The owner reported that she had been missing the baby bottle nipple for over 1 month.

Clinical Differential Diagnosis

Foreign body, gastroenteritis, pancreatitis, intussusception, intestinal torsion, neoplasia.

Sampling

Exploratory enterotomy revealed a baby bottle nipple lodged in the mid duodenum. Mild localized peritonitis was present at the site of obstruction.
