Jejunal intussusception in a 2 year old FS DSH cat

This 2-year-old FS DSH was presented for progressive anorexia and lethargy over the previous week. Physical exam revealed a palpable mid caudal abdominal mass and moderate dehydration. CBC and blood chemistry were normal. Radiographs confirmed a soft-tissue density in the mid caudal abdomen.

Sonographic Differential Diagnosis

Intestinal intussusception.

Image Interpretation

This image demonstrates an intussuscepted segment of small bowel. In the far field, greater than 5 wall layers are identified, supporting intussusception. Bowel wall detail is conserved. The lumen of the centralized intussusceptum is mildly distended, with echogenic chyme. Echogenic omentum is noted in the far field consistent with low grade inflammation. The video demonstrates transition from normal bowel to intussuscepted bowel as the viewer follows the wall layers doubling into the intussuscepted portion of intestine.

DX

Jejunal intussusception

Outcome

The intestinal anastomosis site dehisced 2 days postoperatively, which prompted repeat anastomosis and resection. The patient then recovered uneventfully.

Clinical Differential Diagnosis

Neoplasia, intussusception, abdominal abscess, or lymphadenopathy.

Sampling

Laparotomy revealed a jejunal intussusception that was resected and anastomosed. Histopathology: intussusception with ulcerative, suppurative, and necrotizing enteritis, and chronic bacterial peritonitis.
