Lymphoblastic lymphoma in a 7 year old MN DSH cat

A 7-year-old MN DSH cat was presented for weight loss and poor appetite. Physical examination revealed persistent hyperthermia after a prior treatment of antibiotics. Moderate neutrophilia with a left shift and mild hyperglycemia were the only abnormalities found in the CBC and blood chemistry panel.

Sonographic Differential Diagnosis

These findings are consistent with multicentric neoplasia involving intestine and liver, likely lymphoma. Inflammatory disease, such as complicated inflammatory bowel disease/granulomatous disease is less likely but possible with hepatic abscess or concurrent granulomatous spread. Generalized hypoechoic micronodular parenchyma is noted otherwise.

Image Interpretation

Oblique view of a deep right liver reveals a well-circumscribed, ovoid, hypoechoic, coarse, 3-cm mass within the middle liver. An accompanying mass effect represented by caudal and right lateral deviation of the gallbladder is also present. A concurrent infiltrated portion of small intestine is noted with complete loss of mural detail strongly suggestive for multicentric neoplasia such as lymphoma or carcinoma.

DX

Lymphoblastic lymphoma, liver.

Outcome

The patient was euthanized owing to poor prognosis of lymphoma cell type and organ involvement.

Sampling

23-gauge US-guided FNA of the liver demonstrated lymphoblastic lymphoma.
