-Adult neutered male Mini Schnauzer mix
-4 day history of vomiting (may be a little longer, I’m at home and a little fuzzy on this history)
-non-specific bloodwork, cPLI abnormal. Right limb pancreas looks normal, left appears to have some surrounding inflammation
-Is this gastric wall consistent with gastritis and vomiting? Just want to make sure we’re not dealing with something infiltrative.

I see CPL elevatuions all the time in gastritis and panc looks good… anyone know of any stuidies distinguishing pancreatic and gastric Lipase on the CPL testing? Lipase is produced in the stomach as well and Im wondering if cross reactivity here on some of these cases.
CPL is a measure of
CPL is a measure of pancreatic lipase using an immunoassay test. As pancreatic lipase is structural different from other lipases it would not pick up other lipase activity. The only paper that I am aware that gives false postive CPL is in dogs with Cushing’s disease:
Dogs with HAC had higher SPEC concentrations and more positive SNAP results than clinically healthy dogs with normal ACTH stimulation test results. Specificity of SPEC and SNAP assays in HAC dogs without clinical pancreatitis were 65 and 45%, respectively. Pending further study, SNAP and SPEC results should be interpreted cautiously in dogs with HAC to avoid false diagnosis of concurrent pancreatitis.

Excellent stuff thanks Remo!…. lipase is metabolized in the kidney right??…. I’m wondering if renal dysfunction could cause false elevations as well…

Decreased renal excretion during experimental AKI did not cause consistent and correlated increases in serum Spec cPL, lipase activity, or TLI.
