Two month old kitten with ascites and failure to gain weight

  • 2 month old M Maine Coon X at cat shelter presenting for grade 5/6 holosystolic heart murmur loudest on the right side and chronic constipation currently being managed with lactulose.  The kitten has been checked for parasites and dewormed multiple times.  He is pot-bellied and not gaining weight.
  • No bloodwork performed yet (although I am pretty sure they test all of their cats for FeLV/FIV)
  • Echocardiogram read by Sonopath cardiologist shows a small VSD and slight PA enlargement.  No treatment recommended at this time.

    • Abdominal US shows the gastrointestinal tract to be mild to moderately distended with ingesta and normal GI motility.  The patient was not fasted for the exam. Small pockets of free anechoic fluid are seen. No other abnormalities seen.
    • Is it normal to see multiple small pockets of free anechoic effusion in a 2 month old kitten or do I need to be worried about FIP?  Other rule outs include hypoproteinemia.

Although the cardiac anomalies may be contributing to the clinical signs, would consider FIP, hypoalbuminemia, congenital liver issues first. Analysis of the ascitic fluid would be first diagnostic step.
