Tumour vs muscle hemorrhage

– 7 yr old FS Lab presented w 5 wk history of bruising and a swelling in inner L thigh
– o thought this was an injury and would go away on its own
– blood work: low platelet count (9), moderate anemia, 4 Dx -ve, biochem bilirubin only 1 point above normal
– abd. u/s nsf; this is u/s of the mass and what I think is the enlarged local LN; low colour flow in the mass itself

Looks like tumor to me as there is more organization and well defined feeder vessel than a hematoma would have and little inflammation. 22 or 20g fna is what I would do. i pinged Nele on this as she has more experience than I though.

I absolutely agree with tumor

I absolutely agree with tumor until proven otherwise. Very solid in appearance with disrupted echarchitecture, internal vasculaization, mass effect…. looks not a bit like hematoma/hemorrhage unfortunately. Also the LN presents malignancy criteria with increased short-over-loong axis ratio, decreased echogenicity….

I’d FNA both and would not believe in any other diagnosis than neoplasia. Hemangio likes to do that among others btw…
