Tumor in Right Heart and Pericardial Sac in 2 yr dog

  • 2 yr FS German Shepherd with anorexia, listlessness, abdominal distension
  • muffled heart sounds, BCS 2/5, pendulous abdomen with fluid wave, weak pulses
  • abdominal rads- enlarged liver, loss of serosal margins
  • thoracic rads- very globoid heart silhouette
  • abdominal sonogram- mild hepatomegaly with distended hepatic veins, mild abdominal effusion, no masses in spleen or liver
    • 2 yr FS German Shepherd with anorexia, listlessness, abdominal distension
    • muffled heart sounds, BCS 2/5, pendulous abdomen with fluid wave, weak pulses
    • abdominal rads- enlarged liver, loss of serosal margins
    • thoracic rads- very globoid heart silhouette
    • abdominal sonogram- mild hepatomegaly with distended hepatic veins, mild abdominal effusion, no masses in spleen or liver
    • echo- 2 x 5 cm ovoid echogenic mass moving between the right atrium and right ventricle which appears to extend into the pericardiac sac (10 cm mass) and surrounded by pericardial fluid
    • owner declined pericardiocentesis and FNA of mass

Wow thats a whopper and very young for hemangioracoma. Other types of tumors like fibrosarc or mesenchymal tumor or even lymphoma. Fna is the way to go here or even pericardectomy. Please share the cytology or histopath if you get it.

I obtained several Tru-Cut

I obtained several Tru-Cut biopsies and here is the report.  In retrospect, I should have obtained a sample from the the right atrium via post portem exam also.

NCSU Rollins Lab Histopath report:

Microscope Description: A sample of pericardium and mass obtained by tru-cut biopsy contains pericardium with loss of endothelium, surface fibrin, and fibroplasia with infiltrates of siderophages. Adherent to the pericardium is a diaphanous accumulation of loose connective tissue. There is moderate vascularity and coagulative necrosis with complete loss of cell detail. Few vessels contain intraluminal fibrin thrombi. Areas of hemorrhage can be discerned. The changes extend to the tissue margins.

Morphologic Diagnosis: Pericardium: Fibrosis and endothelial effacement with fibrin exudation and associated necrotic mass.

Comment: The mass contains loose connective tissue with fibrin and hemorrhage but due to necrosis I cannot further characterize the changes. Differentials include neoplasia such as myxosarcoma, and recurrence hemopericardium due to an atrial tear.
