Small echogenic mass in a 13yr old FS CKR SP

  • 13 yr FS Cocker  Sp presented for loss of appetite since 3/22/20. Bucky seems painful -shaking legs when standing and laying down per owner.
  • CBC shows Hgb=12.0g/dL, MCH=15.3 pg, Retic=142.3 K/mcL, PLT=52 K/mcL, pltcrit=0.05%, WBC=10.9 K/mcL, Baso=0.11K/mcL, spherocytes noted.  Chem prof shows ALT=821 10-125 U/L.  USG>1.050.
    • 13 yr FS Cocker  Sp presented for loss of appetite since 3/22/20. Bucky seems painful -shaking legs when standing and laying down per owner.

    • Abdominal ultrasound showed dorsal deviation of the splenic capsule suggestive of possible mass, benign hyperplasia, or extramedullary hematopoiesis.  No FNA’s done today as primary vet concerned about IMHA and coag panel is pending.
    • Abdominal ultrasound also showed this echogenic nodule immediately adjacent to the dorsal caudal aspect of the right kidney and ventral to the CVC.  I believe that this is just an incidental finding but wanted to get differentials on what this might be….my current differential diagnosis list includes lipoma, granuloma, and fibrosis.  It does not appear aggressive or neoplastic at all to me.  It does not seem to be in the right place for pancreas.  Right adrenal was normal.  GI also appeared normal.
