Severely elevated BA, asymptomatic large breed rescue dog.

New rescue, 4-5 yo, 55# Lab Mix Male intact from TX.

HW + Anaplasma +. On Doxy before HW treatment. Giardia +, treated with Panacur.

Chem 10/CBC before neuter: ALT  1377, ALP 811. BA pre 51, post 255! Completely asymptomatic.

U/S: subjectively small liver, small hyperechoic pinpoint foci. Normal PV/AO/CVC ratio. Chronic hepatitis vs other.

Small stones/sand in GB.

Enlarged jejunal LN (5.5mm) due to GI parasites vs other, and medial iliac LN (8mm), reactive (sub-clin prostatitis vs other)

Medullar rim band in kidneys.

Question 1: a weird vessel on R intercostal view, very ventral, not in the liver. (I was looking for an intrahepatic shunt.) Not a shunt, but what vessel is this? I normally don’t see it.

Question 2: Have you ever seen Heterobilharzia americana? I read it can cause weird hyperechoic granulomas in the liver.

Started liver support (metro, ursodiol, denamarin, liver diet). 

Rec liver biopsy when they neuter him.

Looks like it is within the liver and likely a, IH shunt. I would CT it with those BA numbers especially prior to anesthesia
