SAM?

Older cat, recent new heart murmur.

Asymptomatic, was done for pre-anesthetic purposes.

-I think the LVOT is turbulent but I was not able to document SAM on m-mode. Do you see SAM?

-In a case that is thick, but has normal LA:Ao and SAM, do you start on something like atenolol? HR was about 225 during exam period.

I can’t see SAM here. If would recommend increasing your PRF because a low Nyquist limit (0.66 here) catches up a lot of weak signals as well.

If LA:AO is normal, there is not really any evidence that Atenolol really helps. If the heart rate exceeds 240 in an excites cat or 200 in a calm cat and if the vmax across the left ventricular outflow tract is > 4 m/s I usually give Atenolol low dose (e.g. 0,625 mg/cat bid). If the cat is difficult to pill, I would not give Atenolol.
