Hi there –
I have a 14 year old DSH FS cat with a week history of inappetance and weight loss.
- bcs 3/9
- dehydration
- large very firm mass palpable in cranial abdomen
- severe halitosis
No blood work.
Ultrasound shows a large hypoechoic mass stemming from the right kidney. There is mild pyelectasia. The left kidney is also abnormal with loss of corticomedullary distinction and mild pyelectasia.

I wasn’t able to find any images like this in the archive, and I was hoping that someone could give me an idea of ddx. Owner isn’t interested in FNA, chemo, or surgery, so we have started palliative pred and buprenex.

Liz looks like either renal
Liz looks like either renal carcinoma or lymphoma. Palliative pred will help wiht renal lsa but not carcinoma. If its unilateral and no infiltrative patterns in other organs then nephrectomy indicated. I would really push th efna to diff the 2 and see if an attempt at palliative or even pred leukeran may help here.

the first video is the “unaffected” kidney. Looks affected to me so I was betting on lymphoma. What are your thoughts. The owner is no way no how on FNA as we’d have to sedate and there are money issues.

I would lean towards lymphoma
I would lean towards lymphoma as a carcinoma generally only affects one kidney. There may be some improvement with preds. Fluid therapoy is also indicated.
