Possible RV enlargement without RV hypertrophy in HW positive dog

  • 13 month old FS terrier X adopted from shelter in January – originally from Mississippi.  Proheart was administered in January.
  • Suffered episode of acute collapse while on vacation in California about 5 days ago. Presented to ER 2 days later for lethargy and diarrhea. CBC and mini chem panel performed by ER showed eosinophilia (2.45 K/mcL) and AST=62 U/L.  Fecal was positive for Giardia. HWT was positive.  Radiographs show possible cardiomegaly.

    • Echo performed today shows all normal cardiac measurements.  Doppler studies are all normal except for a trace TVI (<1.0m/s) seen only on left parasteranal RA/RAU view.  HR is wnl at 72bpm.
    • While I do not see any right ventricular hypertrophy, on RP transverse views, the RV and RVOT appear enlarged.  Can you have RV chamber enlargement with HW dz without TVI, PI, or RV concentric hypertrophy?  Is this real or am I off angle? And is this significant in terms of treatment planning?

    No septal flattening seen.

Looks like a big right heart.

Looks like a big right heart. Check hepatic veins for dilaton usually clinical rt heart disease will show HV/CVC congestion as the right heart fails. TR jets can be really small and tough to line up on for precise doppler if the TV is in good shape so you may be underestimating. The HV/CVC check will tell you how hard you need to chase TR velocities that are tough to quantify. Paroxysmal arrythmia possible too would need a Holter, which we have as well with cardiologist interpretation. Just email info@sonopath.com if needed.
