perirenal abscess versus mass


This kidney is surrounded by abnormal tissue and fluid. I hope to be able to aspirate it, and if so, I would like some pointers as to how to approach it (go through retroperitoneal space? or just wherever I can get it close to the skin?) And, if I manage to aspirate something that looks like pus, what could I infuse? There is a postitive urine culture with E.coli but I don’t have senstitivity on it yet).

    It is sensitive to baytril, seems like a good choice but how much? Dilute it before infusing?

  2. No do not infuse abs into

    No do not infuse abs into renal abscesses. Pancreas prostate liver mesentery lung all good but not into kidneys.

      A colleague did this once and sent it into terminal renal failure and this looks more like a tumor which may have uti as well. I would 25g fna the cortex and drain any fluid and culture and tx for infercion til you ride out the cyto read.
