Lung mass in a couging miniature poodle

  • 8 year old mn miniature poodle.
  • Chronic cough, nonresponsive to antibiotics.
  • Chest radiographs show radio-opacity in the region of the right middle lung lobe.
  • Abdominal ultrasound shows a 3.1cm x 3.4cm round  mass adjacent to the right ribcage in the dorsal lung field between the 7th and 8th ribs.
    • 8 year old mn miniature poodle.
    • Chronic cough, nonresponsive to antibiotics.
    • Chest radiographs show radio-opacity in the region of the right middle lung lobe.
    • Abdominal ultrasound shows a 3.1cm x 3.4cm round  mass adjacent to the right ribcage in the dorsal lung field between the 7th and 8th ribs.
    • Fine needle aspirates done using a 25 gauge needle, 22 gauge needle, capillary, corkscrew, and syringe aspiration techniques did not yield much.  Slides stained in house just showed hyaline material.  Cytlology is pending on the only slide with possible yield.
    • Any ideas what this could be based upon sonographic appearance?
    • My rule outs include bronchogenic adenocarcinoma, lobar consolidation, lobar pneumonia, lung torsion.

Your rule-outs are spot on

Your rule-outs are spot on but also consider a granuloma. Carinomas generally speaking can be diagnosed on FNA. In this case with the non-diagnostic FNA torsion, consolidation, and granuloma are possible. Consider doing a bronchoscopy with BAL if there is no torsion. CT may also required, especially if surgery is being considered.

Bornchogenic carcinomas dont

Bornchogenic carcinomas dont exfoliate as well as adenocarcinoma but if you are comfportable with it a core bx of the parenchymal portion of the mass should yoield the dx as its in the structure of the pathology. Otherwise if localized these things only resolve with sx anyway so thoracotomy.
