Hello,

I ve scaned this Italian Greyhound that was HBC and I would like to get your expert opinion on :

1. If ALT is 3000 and Bile Acids are 140 does the Bile acids relates to trauma or I should think a preexisting condition? ( I know it does not change the management)

2.I was wondering what is the hyperechoic “structure” inbetween dyaphragm and liver. I suspect is a clot / trying to form a clot? I ve seen this before in abdominal bleeding ( Hemangio/ Splenic mass).

3. Is it a liver lobe rupture or just me .

There are increased portal

There are increased portal markings so I think there is some level of chronicity here. Free fluid could be transduate from portal hypertension so sample it to be sure its hemorrhage. Clots usually are hypoechoic first til the organize and they become hyperechoic.

HBC sometimes have underlying disease… i.e. wandering dog with lepto or mushroom toxicity which would fit the profile you mentioned and then also HBC in addition to liver hypoxia and trauma causing the LEs to elevate. Hematomas in the liver will resolve so rescanning periodically will tell the tale on this one ad rerunning the LEs as well as sample fna the liver and tap the fluid to see its character to ensure not just a transudate.

I did sample it and it was
I did sample it and it was active bleeding with a high PCV of 40%.

Repeat the ALT and BA in 2-3

Repeat the ALT and BA in 2-3 days and if going down most likley it was up from the trauma.
