Shya is a FS 7 yr 12# chihuahua who has a past history of GME diagnosed a few years ago at WSU vet school and has had ongoing but stable mild neuro deficits in rear limbs. Recently she presented to the ER with a history of ADR and retching. Owner reported she had voided not long before presentation and the ER vet noted a very distended bladder and catheterized to remove urine and did not have any problems passing catheter. Reportedly removed 240 ml of urine. Pet presented to rDVM with history of unable to void urine. There is a purplish discoloration swelling just inside the vulva and rectal palpation of the urethra was unremarkable except for possible thickening at the distal aspect of the urethra. The pet was so small a vaginal scope exam was not done so tried to image this area via the perineal area between the anal sphincter and vulva. I thought I was seeing the urethral papilla and thought it looked normal. I am wondering if this is a neurologic problem and not a mechanical obstruction but the rDVM thinks the urethra may have been traumatized during the catheterization. A 5 fr red rubber catheter was reportedly used. Pet has been on chronic prednisone for the neuro issues previously diagnosed. The bladder was not able to be expressed manually.

Any other thoughts are appreciated.

Urethra looks a little thick but not dramatic but I would certainly get histopath in case tcc starting here… traumatic catheterization? Sounds like its likely time to repeat the x sectional imaging of the T-L and L-S spine to assess any progression for the general issues and abdominal sonogram for the vomiting issue. Urine culture and coverage for UTI as neuropathic urinary patients get uti often.

I wonder if this dog has a problem with dyssynergia from the back issues. I am sure there could be urethral trauma in such a little girl. Like EL said a urine MIC would be a good idea to RI/RO cystitis – especially been on steroids. In male dogs repeat catheterizations can be utilized  but not practical in a female. Just another thought.
