Is this always a surgical
Is this always a surgical case? What factors do you use to help decide when it is surgical vs. medical? I can’t say I’ve seen many cholecystectomies or CBD flushing done on cats in this area.

Its an underdiagnosed
Its an underdiagnosed condition and gets treated for simple cholngiohepatitis/pancreatitis and they wax and wane. CBD > 0.5 cm with clinical signs and LEs up is considered surgical. Only 0.2 cm stones typically will pass and so there is this low grad malaise these cats have in my experience… then they get cleared out surgically and they do great… just like occult mucocele in dogs running “subclinical.” I’ve hadd a few respond really well to actigall and then most others it does nothing… very unpredictable medical response both in dogs and cats with bile stones and actigal and cholhep tx.
