Hello,

8 year old Bichon X M/N dog presented to referring vet for dental cleaning. Elevated ALP and ALT found on pre-anesthetic bloodwork. Blood was sent away for confirmation to Idexx. ALP 532 (5-160) and ALT 304 (18-121) and bile acids pre 56 (0-7) and post 31 (0-15). Dental cleaning was put on hold pending ultrasound. No signs of illness other than ADR and diarrhea about a month, which has since resolved.

Ultrasound – small amount of mineral within lumen of bladder (will be following this up at rDVM), NSF with kidneys/spleen/liver/GB. BUT I did find some interesting stuff while cruising around the adrenals. Left adrenal measured 0.5cm and just caudal to renal artery/vein I found two separate structures seen that were hypoechoic to anechoic and were multi-septated. Cranial measured 1.29cm x 2.06cm and caudal once 1.38 cm x 1.18cm. Between them there was blood flow as a branch off aorta. There was a similar structure on the right side. However, I never did find the right adrenal but in the area I would have expected to find it there was something that was hyperechoic and shadowing.

These multi-septated structures do not appear to be lymph nodes. Other than cysts, what else should I be thinking? Etiology for cysts in this area?

There are often LN in this

There are often LN in this region aortic or renal and when chronic lymphadenitis occurs they can degenerate and create a bag of fluid/lymph when you aspirate them….. Happens in ibd cats a lot at jejunal LN. Can say more based on the image

“? R” – starting sag at level of R adrenal moving caudal to the structure

“R adrenal?” – starting at R adrenal in trans before turning to sag. In this clip there is a bit of hyperechoic mildly shadowing in area where I would have expected the adrenal

“L adrenal/?” – start sag at level of L adrena and moving caudal to the two structures. There was blood flow in between the two structures and they were definitely separate structures.

Yes these are cystic lymph

Yes these are cystic lymph nodes. If you aspirate them you will get transdudate or lymph or somethign in between. Not pathological. Beautiful images:)
