Cystic calculi or only sediment?

10yo MI Lab with diagnosed leishmaniasis (controlled at the time)

 

Came in with history of hematuria (1 or 2 drops of fresh blood in the beginning of the micturition). Did an u/s which revealed bilateral parenquimal prostatic cysts and what seems to be free shadowing sediment in the dependent portion of the bladder.

UA: no bacterial growth, no cristals, just blood and epitelial cells

Abd x-ray: no visible calculi

I’m very confident that this dog has sediment, but I can’t get a good image of sediment suspension, no matter how hard I agitate the bladder. Just a little tiny bit of speckling, and that’s all. The shadowing sediment remains on the dependent portion of the bladder in left, dorsal and right recumbency.

What do you think?

No worries JG we don’t make fun of anyone as we have all had learning curves and if we are smart we continue to have one:)

This is bladder sand (distinct flat sand, acoustic shadowing as a grouping) and given that he’s an in tact male with prostatic cysts its likely struvite associated with UTI whether you culture it out or not. If he has prostatomegaly and the owenrs won’t neuter you can try finasteride and baytril and ideally culture the urine and prostatic cysts. Assuming baytril is effective on C/S then treat him for a month and rescan and culture and see where you are regarding the sand. usually neiutering th ein tact makes and treating uti/prostatits resolves the sand as well on these guys but usually takes 4-6 weeks post neuter.

Yep, we’re going to neuter him and already started on baytril. I just wasn’t 100% absolutely sure about the sand and wanted to rule out the possibility of having a stone before the cx.

You bet.. it should resolve

You bet.. it should resolve medically ones your clear the prostatic urethra plumbing and treat any infection avoiding cystotomy. If solid stones I would say cystotomy.
