Cut-off for feline spleen thickness?

  • 13 yr FS cat
  • Weight loss
  • Anorexia
  • Hypercalcemia (12.7)
  • Rest of BW WNL

I surprisingly didn’t find much.  No LN, liver and GI looked WNL.  The only thing was a spleen that measured close to 1 cm…. 0.96 cm, 0.83 cm.

How far over (or under) the 1 cm “cut-off” do you think is potentially an issue?

I started the cat on Pred-L, the O wanted to do Something.  If there is MCT or LSA in the spleen, do you think that would change the thickness in a recheck US?

Thanks!  Karen

Hi Karen. I’ll let one of the specialists comment on the spleen, but our protocols for SonoPath Mobile dictate that if nothing definitive is noted in the abdomen, and the animal has hypercalcemia, scan the thyroid and parathyroids also.

Thank you!!  Makes sense, and did not even occur to me…..  I can have the cat come back in and do that.  I may need to watch a few videos first.  🙂

A higher frequency probe is

A higher frequency probe is required; we scan all thyroids and parathyroids using a linear probe, and in both long and short axis.

If you are a SonoPath member, you can go to the basic search engine and type in the search box:

thyroid parathyroid

and it will bring up lots of cases with images and videos, which will give you the anatomical landmarks.

For a more instructional experience, you might want to attend our SDEP clean-up lab Nov 30-Dec 2 in Andover NJ

That hypercalcemia range is typical of feline idopathic hypercalcemia… write up is in the curbside guide if you have it

Rule of thumb on cat spleens is < 1 cm width (ensure you arent oblique when measuring). In a clinical cat wiht a spleen > 1 cm especially if scalloping contour then I 25g fna. Splenitis, reactuve spleen, round cell neoplasia are iusually those diffs in those cases.

On your image set nothing bothers too me much but wiht the pred the spleen will drop size nearly immediately so may wan to fna anyway.

Maldigestion panel, chest rads and full cns exam would be my next steps here.

Catlady…. Thank you for the

Catlady…. Thank you for the description!  I can do that.

And I am interested in the clean-up lab in the fall…. need to see if I can get there.

EL… thank you!  Good point on making sure view is not oblique when measuring.

I do have the Curbside Guide, and love it!  Was reading the Hypercalcemia section yesterday.  🙂

The O will be thrilled if it is Idiopathic Hypercalcemia.  That was her “Dr. Google” diagnosis when she dropped the cat off for the US…. lol.
