The cystic component and
The cystic component and significant disorganization would suggest biliary carcinoma. Biliary adenoma with remodleing possibel too need to see if resectable as thats the only real treatment. Patients get clinical from space occupying issues or portal hilus issues in my experience. As you can see these masses can get really big and just move caudally where they can.
