this is a 15 years oild icteric cat that was diagnosed with chronic pancreatitis, cholangiohepatitis .despite intensive in clinic treatment for 1 week there is no improvment on liver values/ clinical . The CBD measures 1.55 cm ( was 1.2 cm 10 days ago )
I m wondering if there is actually a mass at the level of DPAP ? I shouldn’t have any collor flow inside CBD unless overlaping or mass, right ?

Mineralizing cbd mass suspect carcinoma wiht post hepatic obstruction. Chronic inflammatory polyp possible but less likely. If no mets then needs cholecystoduodenostomy but liver mets on these lesions can be very isoechoic so use hi frequency provbe screening the liver.

Thank you EL. Unfortunately cat didn t make it. Lots if mild hyperechoic nodules on the liver as you suggested. Radiologist considered them reg nodules??? histo pending.
Cats don’t get regenerative
Cats don’t get regenerative nodules typically until really old so its possible I guess. Dogs get them very frequently. Over the years I have considered any nodule in a cat liver suspect of pathology until they get really old then they can do what they want:) This approach has panned out pretty acurate over the years… cat nodules need needles if slightly hyperechoic isoechoic or hypoechoic… The very hyperechcoic nodules are usually fatty nodules but again mainly in older cats. With a cbd mass and nodules the nodules are mets til proven otherwise and a needle proves them one way or another. But If I saw nodules in this cat with that cbd mass I would have done multiple fnas right there with cyto by the next day and then surgery if the cyto came up benign. Then inspected and sampled liver anyway at surgery. This doesnt waste anyone’s time or money and certainly does the right thing for the suffering cat.
