Suspected insulinoma in a 9 year old FS Labrador

A 9-year-old spayed female Labrador dog was presented for evaluation of persistent hypoglycemia and panting with an insulinoma being suspected.

A 9-year-old spayed female Labrador dog was presented for evaluation of persistent hypoglycemia and panting with an insulinoma being suspected.

Sonographic Differential Diagnosis

Left pancreatic nodule adjacent to the transverse colon. Strongly consistent with insulinoma, pancreatic adenoma is suspected. Carcinoma is possible. No obvious metastatic disease.

Image Interpretation

The left pancreatic base revealed a hypoechoic nodule with mild irregular contour measuring 2.6 x 2.25 cm. This was adjacent to the transverse colon. The remainder of the pancreas appeared normal in the right base and right limb along the duodenum. This appears to be a focal lesion from a macroscopic standpoint. Power Doppler assessment of the nodule presented only minor vascularity.

None

Comments

Surgical intervention is recommended with direct inspection of the liver, hepatic lymph nodes and regional lymph nodes to assess for possible micrometastasis. 

Clinical Differential Diagnosis

Insulinoma, neoplasia, chronic liver disease, Addison’s disease

Sampling

None. However, ultrasound-guided FNA or core biopsy could be performed for confirmation or direct exploratory surgery.
