RAD – Normal thoracic age-related changes in a 6 year old FS Chow dog

The patient is a 6 year old FS canine Chow dog presented with 3 episodes of collapse this morning between 6-7:30 am; the patient is relaxed, aware and responsive throughout the episode but legs are splayed out. Not precipitated by exercise. Lasts about 1 min, no vocalization/urine/defecation. O thinks she is completely normal by the time of exam 2 hours later.

Physical Exam: BCS 6/9 Cardiovascular/respiratory wnl on auscultation. No neurological deficits. Ambulation normal.  Exam is wnl overall.

Image Interpretation

RAD – right lateral and VD thorax, right lateral abdomen – Osseous structures:
Overall mild degenerative changes were associated with the axial sceleton including
emerging spondylosis at T4/5.
Intrathoracic structures:
The degree of inspiration was fair.
There was a redundant tracheal membrane within the cranial thoracic trachea which is
an incidental finding. Otherwise the mediastinum and pleural space were within
normal limits. The lung revealed a mild generalized bronchointerstitial pattern. The
cardiac silhouette was within normal limits regarding size and shape. The major
vessels and pulmonary vasculature were within normal limits. There was no sign of
vascular congestion.
Intraabdominal structures: 
The serosal detail was within normal limits. The liver extended beyond the costal arch
but the liver margins were pointed. The spleen was within normal limits. The kidneys
and urinary bladder were within normal limits. The stomach was contained a moderate amount of gas and fluid. The small intestinal loops were non dilated and of even
diameter. The colon was largely empty.

DX

Normal thoracic age-related changes

Outcome

The moderate aerophagia is likely to be a function of stress or pain. There is no
radiographic finding supporting respiratory distress as being the reason for it.
The impression of hepatomegaly is likely to be a consequence of normal age related
laxity of the hepatic ligaments. A vacuolar hepatopathy – or even less likely – diffuse
inflammatory or neoplastic infiltrate cannot be ruled out entirely. Further definition by
ultrasonographic examination and sampling is only required in case of relevant liver
enzyme and/or bilirubin elevation.
