Multfocal biliary mineralization in a 16 year old MN DLH cat

A 16-year-old neutered male DLH cat with a history of liver and gall bladder disease was presented for evaluation of weight loss and hair loss. Abnormalities on CBC and serum biochemistry included severe leukocytosis (51000), neutrophilia, elevated total bilirubin, mild hyperglycemia, and elevated ALT (207), ALP (141) and GGT (33) activity.

 

Sonographic Differential Diagnosis

Multiple common bile duct, cystic duct, gallbladder, and lobar biliary calculi. Enlarged pancreas.

Image Interpretation

The liver was riddled with multiple lobar biliary calculi; a common bile duct calculi with strong shadowing was noted at the duodenal papilla. The “too many tubes” sign was noted in the cystic duct due to tortuousity and multiple gallbladder calculi. A minor amount of free fluid was noted between the liver lobes. The pancreas was enlarged and the pancreatic duct was dilated at 0.46 cm. This is likely secondary to post hepatic obstruction.

Hepatic disease: cholangio-hepatitis complex, lipidosis, neoplasia, FIP, granulomatous disease, abscessation; Gall bladder disease: cholecystitis, neoplasia, cholelithiasis; Pancreatic disease: pancreatitis, neoplasia; Peritonitis.
