Left adrenal in this patient created a mass 4.57 x 1.37 cm cranial; 1.48 cm caudal with phrenic vein invasion as well as invasion of the vena cava to the level of the liver; invasion is 4 x 2 cm. Expansion of the vena cava also noted. A large lipoma was noted in the pelvic inlet. The prostate was severely enlarged, nodular and irregular
The immediate difficulty defecating is likely owing to the lipomatous-type mass in the pelvis. However, vague clinical signs are likely owing to the adrenal mass which is most consistent with pheochromocytoma; possible adenocarcinoma. Serial blood pressures are recommended. 3-view chest radiographs are also warranted if not already performed.
