Case Study

Interstitial cystitis with embedded mineralization in a 7 year old FS DSH cat

This 7 year old FS DSH cat presented with hypercalcemia and vomiting.

CBC/Chem: glucose 52, Ca 11.7

Urinalysis:  1.041, 3+ blood, pH 7.5, white count 2-5, red count 75-100, struvite crystals.

Interstitial nephritis pattern with imbedded mineralization

Image Interpretation

The urinary blader presented two sepearate areas of mineralization measuring 2.1 cm that appear to be embedde within the bladder wall and not free moving. The bladder wall itself was concentricallh thickened 0.42 cm at mild repletion consistent with interstitial nephritis.

Outcome

No evidence of neoplasia.
Dystrophic mineralization of the bladder wall is an occasional finding in cats and likely secondary to underlying mural disease. Recommend full thickness bladder wall biopsies and removal of the mineralization. Likely resection is necessary as this is likely irritative, but also more likely involved with primary mural disease. The biopsy results of the bladder are important in this patient for further long term management. Differentials of the bladder wall include interstitial cystitis and minor potential for underlying bladder lymphoma.
