Insulinoma in a 7 year old FS Cocker Spaniel dog

A 7-year-old FS Cocker Spaniel was evaluated for suspected insulinoma as she was showing hypoglycemia and seizure activity. Urine specific gravity was isosthenuric. On CBC leukocytosis, neutrophilia, and monocytosis were present.

Sonographic Differential Diagnosis

Left caudal pancreatic nodule with concurrent mesenteric root lymphadenopathy. This is likely insulinoma with local mesenteric root lymph node metastasis. Fine-needle aspirates were performed of the nodules without evidence of complication. Given the patient’s profile pancreatic insulinoma and lymph node metastasis is likely in this patient.

Image Interpretation

The midabdomen in this patient presented a 2.67 x 1.87 cm lymph node mass with micronodular changes. This is strongly suggestive for metastatic disease. Focal mineralization was also noted. This is strongly suggestive of carcinoma. The region of the caudal pancreas presented a separate mass in the left limb. This measured 2.3 x 1.6 cm. A separate pancreatic nodule was noted and measured 0.79 x 0.69 cm with ill defined parenchyma and pericapsular inflammatory, hyperechoic changes. Given the patient’s clinical presentation this is likely an insulinoma. However, given the lymph node metastasis, surgery is not likely an option unless debulking is desired. Fine-needle aspirates of the lymph node and pancreatic nodule would be recommended.

DX

The pancreatic nodule was confirmed to be insulinoma.
